Medical Authorization: I understand that if a medical issue arises while my child is in the custody of Apex Performing Arts staff, I authorize APAC staff to seek medical attention for my child at their discretion. I understand that I (as the parent or guardian of this student) will be responsible for any resulting charges. I, as the authorized parent or guardian of this student, hereby authorize APAC to seek competent medical care for the student so as a condition of participating in this class or camp.

Liability Release: I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of performing arts, and utilizing a dance studio, rehearsal space, or theater. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of my child participating in APAC programming, from which liability could accrue to Apex Performing Arts Collective, it’s officers, agents, employees, instructors, subsidiaries, parent corporation, and all affiliated entities (hereinafter collectively referred to as “Apex Performing Arts Collective”).

I hereby agree to release Apex Performing Arts Collective and hold Apex Performing Arts Collective harmless of all liability and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of participating in programs or renting studio space.

I am aware that this is a release of liability and an acknowledgment of my voluntary and knowing assumption of the risk of injury. I agree to these conditions voluntarily and of my own free will in exchange for the privilege of participation.

Media Authorization: I understand that photos, videos and other media will be used and any recording or image will remain the property of Apex Performing Arts Collective. I (as the parent or guardian of this student) hereby grant permission for the use and retention by APAC of all media formats, as a condition of participating in this production, class, or camp. Usage includes, but is not limited to, print media, social media, website and marketing materials, and may include performers’ faces and names.

All Sales are Final: I understand that Apex Performing Arts Collective is not required to offer refunds on any programming, and that all sales are final. Dismissal from the production or program due to misbehavior or any violation of Apex Performing Arts Collective’s Code of Conduct, or removal from the program early by the parent or child’s own accord, does not entitle the participant or payee to a refund, but may be offered at the program director’s discretion.

Class Size: I understand that Apex Performing Arts Collective requires a minimum of four (4) enrolled students to hold each class or camp. If less than four (4) students are registered before the class or camp start date, APAC reserves the right to cancel the class or camp, and will offer a full refund to all enrolled students.
